The Most Common Autoimmune Biomarkers Explained
There are more than 100 autoimmune diseases, and they affect the body in different ways, which is why there is no universal autoimmune panel lab test that can diagnose every condition. Instead, healthcare providers typically select which of the following biomarkers to test for based on the person’s symptoms, medical history, and suspected diagnosis.
Antinuclear Antibody
The antinuclear antibody (ANA) test is one of the most commonly ordered autoimmune screening tests. ANA detects antibodies that target structures within the nucleus of healthy cells.
A positive ANA result may suggest an autoimmune condition such as lupus, Sjögren’s disease, systemic sclerosis, or mixed connective tissue disease. However, ANA testing has important limitations. As a result, some healthy individuals have positive ANA results but don’t develop an autoimmune disease, and the inverse can be true as well—some patients with autoimmune disease may initially test negative. For this reason, ANA is often used as a starting point rather than a definitive diagnostic test.
C-Reactive Protein
C-Reactive Protein (CRP) is a protein produced by the liver in response to inflammation. Elevated CRP levels can indicate that inflammation is occurring somewhere in the body.
Because inflammation has many possible causes, from infections and injuries to chronic diseases, CRP cannot determine whether inflammation is specifically related to an autoimmune condition. Instead, it helps healthcare providers evaluate overall inflammatory activity, monitor disease progression, and gauge treatment response.
Erythrocyte Sedimentation Rate
The erythrocyte sedimentation rate (ESR), sometimes called the sedimentation rate or “sed rate,” is how quickly red blood cells settle, making it another general marker of inflammation. Like CRP, an elevated ESR does not diagnose autoimmune disease. Instead, it provides another indicator that inflammation may be present and is often interpreted alongside other laboratory findings.
Disease-Specific Autoantibodies

Even disease-associated autoantibodies have limitations. Depending on the disease, some people may have detectable autoantibodies before symptoms develop, while others may have an autoimmune disease without the antibodies commonly associated with that condition.
What Your Blood Work Can and Cannot Tell You
Laboratory testing provides valuable information, but every result must be interpreted within the broader clinical context.
Positive Results Do Not Always Mean Autoimmune Disease
Some autoimmune biomarkers indicate that the immune system has produced certain antibodies or that inflammation is present. However, these findings alone do not confirm an autoimmune disease.
For example, positive ANA tests occur in a substantial percentage of healthy people. Likewise, elevated CRP or ESR may reflect infection, injury, or another inflammatory condition rather than autoimmunity.
Healthcare providers consider whether laboratory findings match a patient’s symptoms before making a diagnosis.
Normal Results Do Not Always Rule Out Autoimmune Disease
The opposite situation can also occur. Some patients experience autoimmune symptoms despite normal laboratory results, particularly during the early stages of disease.
Certain biomarkers may not become detectable until later, while others fluctuate as disease activity changes over time. Therefore, one of the most important tips for getting a diagnosis of an autoimmune disease is getting multiple opinions and being open to repeat testing—don’t hinge everything on a single blood draw.
The Whole Clinical Picture Matters
Rather than relying on one laboratory value, physicians combine several pieces of information, including:

Understanding Seronegative Autoimmune Disease
People who experience clear autoimmune symptoms despite negative or inconclusive blood work may have seronegative autoimmune disease. The term “seronegative” simply means common blood tests cannot detect the antibodies typically associated with a particular condition.
While seronegative disease is more common in some autoimmune conditions than others, it highlights that laboratory testing is only one part of the diagnostic process.
Why Blood Tests Can Be Negative
Negative or inconclusive results can occur for several reasons. Not every autoimmune disease has a highly sensitive or disease-specific biomarker, and some people with established autoimmune disease do not have the autoantibodies typically associated with their condition.
